Calvary’s Hospice and Palliative Care services are designed to meet and address the physical, emotional, and spiritual needs of patients and their loved ones.

CalvaryCare® is now available to Queens residents at The Ozanam Hall Nursing Home in Bayside. Our patients there receive the same hallmark care that that patients receive at our main facility in the Bronx. Under the oversight of Calvary Hospice, patients receive short-term inpatient care at Ozanam Hall. The care is intended for patients whose symptoms and care needs cannot be met at home.

Admissions Requirements
To be considered for CalvaryCare℠ at Ozanam Hall, patients must:

  • Be eligible for Hospice inpatient level of care
  • Require inpatient care for symptoms and care that cannot be managed effectively in another setting
  • Need short-term inpatient care

Personalized Assistance
The Community Outreach staff is available to answer any questions or concerns you may have regarding Calvary’s services and to assist in the referral process. Criteria for admission can be clarified by calling an Outreach Liaison Nurse.

Referring Patients
Hospitals, physicians, nurses, and other health care professionals can make referrals or inquiries directly to Calvary Hospice. We encourage families of prospective patients to contact us with any questions.

We offer on-site evaluation by a Calvary clinician.

The following program of care is available to Calvary patients at Ozanam Hall:

  • Calvary Hospice Medical Director (Board Certified in Hospice and Palliative Care)
  • Hospice and Palliative Care trained nursing, Social Work and pastoral care providers
  • Six newly renovated private rooms with private bathrooms
  • Complimentary cable television and telephone service
  • Overnight accommodations for family members in patient rooms
  • State-of-the-art patient care equipment
  • Bereavement Support Program

Religious Services
Ozanam Hall has a multi-faith chapel.

Visiting

There is a family room with a refrigerator, microwave oven, and coffee maker.

Throughout their entire lives, our parents sacrificed everything for us.
Our greatest gift to them during their final days was Calvary. True to the culture of proud Italian-Americans, our parents worked hard all of their lives so that their five sons could have the education and opportunities that they didn’t. So it was only natural that when dad had terminal cancer, then years later mom, uterine cancer, we took them to only place on earth that could relieve their pain and suffering with the most expert end-of-life care – Calvary Hospital – in the place they loved the most – Brooklyn.
