Community Health Needs Assessment (CHNA)
Calvary’s community health needs assessment (CHNA) identifies and analyzes our community’s healthcare needs and issues and helps Calvary prioritize, plan, and act upon them for the benefit of our community. Meeting IRS requirements Section 501(r)(3)(A), Calvary conducts a community assessment every three years, adopting an implementation strategy to meet needs identified through the CHNA.
Calvary makes its CHNA reports available to the public here on our website. Paper copies of our CHNA reports are available for public inspection upon request and without charge. For a copy, please contact:
